Leksell Gamma Knife is now known as Gamma Knife Esprit. The basic concept remains the same. However, now it has the abilities of fractionation beyond typical frame-based treatment. Over the years, Forward planning had remained the identity of Gamma Knife planning, but now Inverse planning has developed as well.

Esprit is equipped with integrated stereotactic Cone Beam CT (CBCT) imaging and high-definition integrated stereotactic CBCT Intrafraction motion management system, which monitors the patient in real time. Motion Management (HDMM) continuously controls the dose delivery.
Esprit provides several workflow options and adaptation possibilities to personalize each individual treatment. The system enables multiple or single sessions, frameless and frame-based immobilization, always with the same high level of accuracy. Elekta Esprit is a platform for the future, ready to support new innovations through improvements to hardware and software.
The reloading of Cobalt source after its half-life of 5.27 years is a major expense after the initial high cost of the system. It affects the viability and slow progress in countries which have no government or private insurance.
